The short answer
You need bone grafting when there is not enough jawbone volume or density to
hold an implant securely. Whether that applies to you can only be established
with a 3D CT scan and clinical examination. Many patients need no grafting at
all; others need a small amount placed at the same time as the implant.
Why bone is lost in the first place
The jawbone is maintained by the forces of chewing transmitted through tooth
roots. When a tooth is removed, that stimulus disappears and the bone in that
area gradually resorbs. This is why:

Does grafting always mean a longer treatment?
Not always. Small grafts are often placed at the same time as the implant,
adding no extra visit. Larger reconstructions may need to heal for several
months before implants can be placed, which can extend the overall timeline —
and, for international patients, may mean an additional trip.

infection, incomplete integration of the graft material, sinus membrane
